To submit a request for testing, to host an inspection or exam, or to inquire regarding evidence or sample collection/storage, please complete the form to the left below.  If you are requesting information regarding an inspection or exam to be held in our laboratory, please check the calendar to the right before requesting dates.  If the date for your inspection or exam is fixed and the date is listed as taken, submit your inquiry for that date and we will follow up with the currently scheduled party to confirm that they still need their reservation.
